Pre-Retirement
- Benefit Estimate – Request Form — Request a benefit estimate from the Trust.

- Vital Statistics Records Address Information — Information on where to write to obtain birth certificates and other forms.

- Section 415 Limits for 2019- Payable as a Single Life Annuity for “Grandfathered Participants” — This table gives the 415 limitations for 2019. The table shows the maximum monthly benefit allowable under law based upon the age at retirement.

Retirement Applications & Related Forms
- Retiree Information Pamphlet — Pamphlet of information to help the prospective retiree file for retirement.
- Preparing for Retirement — This document provides tips on effectively planning for your retirement.
- Application for Benefits [Form 1] — Apply for your retirement benefits. This form includes a description of each of your benefit options.
- Withholding Certificate for Periodic Pension or Annuity Payments [Form 2] — Use this form to tell the Plan the correct amount of Federal income tax to withhold from your payment(s).
- Final Hours Letter [Form 3] — Form for employer to report final employee hours.
- Termination of Employment Certificate [Form 4] — Form certifying termination of employment, to be filled out by you and your employer.
- Authorization for Direct Deposit of Pension Benefits [Form 5] — Authorize the direct deposit of your pension benefits to a specified financial institution.
- Suspendible Employment Determination Form [Form 6] — Describe employment or contemplated employment by an employer who does not contribute to the Plan on this form.
- Beneficiary Designation (if married can’t pre-designate) [Form 7] — Designate beneficiaries to whom your benefits will be paid in the event of your death.
- Medical Plan Self-Payment Billing Form for Retirees Not Eligibile for Medicare [Form 8] — Use this form to elect coverage under the Medical Plan as a retiree.
- Medical Plan Self-Payment Billing Form for Medicare Eligible Retirees [Form 8] — Use this form to elect coverage under the Medical Plan as a retiree.
- Retiree Eligibility Rules — This notice explains the rules governing whether the individual is eligible for medical and prescription drug benefits; dental benefits, if the individual is age 65 or older; and, to a limited extent, life insurance.
For Those Already Retired
- Authorization for Direct Deposit of Pension Benefits — Authorize the direct deposit of your pension benefits to a specified financial institution.
- Benefit Resumption Following Covered Employment — If you return to work and, after your covered employment ends, you wish to resume your retirement benefit, please complete the Resumption of Benefits Following Covered Employment form. In addition, please provide a copy of your termination slip from the employer.
- Benefit Resumption Following Non-Covered Suspendible Employment — If you return to work and, after your suspendible employment ends, you wish to resume your retirement benefit; please complete the Resumption of Benefits Following Non-Covered Suspendible Employment form. In addition, please provide a copy of your termination slip from the employer.
- Suspendible Employment Determination Form — Describe employment or contemplated employment by an employer who does not contribute to the Plan on this form.
